Family Coach- Family Stability Center at Gaston Park

The Family Stability Center at Gaston Park, a Knowledge Quest program, seeks to hire a Family Coach
functioning in a supportive role by assisting families facing parenting challenges by focusing on reducing stress
and trauma through group and individual counseling sessions. Works collaboratively with the Family Stability
Initiative (FSI) staff using parent-centered, strength-based strategies. Has the ability to create trusting and effective relationships with parents, some of whom have legacies of generational trauma. Job assignments will involve local travel.

Key Responsibilities

• Conduct preliminary program enrollment interviews and initial assessments.
• Provisions of ongoing case management and support to families served based on program
standards and requirements.
• Completion of timely written assessments and other required file documentation
• Develop individual CARE plans based on the Arizona Self Sufficiency Matrix for assigned
families.
• Assist in the planning and execution of FSI trainings, parent meetings and workshops,
Family Community Circles and other events designed for the engagement of families.
• Facilitate regular follow up calls, home and office visits and basic case management
functions to new, active and after care clientele.
• Maintain efficient and complete case records as required by agency and by all licensing
bodies.
• Guide clients in development of personal/family goals and action plans to reach those
identified goals.
• Enter and track required program data and evaluation.
• Attend meetings, workshops and trainings for professional development as required.
• Participate in regularly scheduled individual supervision with FSI Program Director
• Cultivate community resources with current and potential community stakeholders.
• Support the monitoring and supervision of program participants by maintaining order and
ensuring safety.
• Engage with existing community partners as well as in participate in the process of
exploring and identifying new community partners and resources for the purpose of client goal
attainment.
• Perform other duties and responsibilities as assigned or directed.

Training and Professional Development:

• T2 Certification in Transition to Success Model
• Nurturing Parenting (or other Knowledge Quest-approved parenting curriculum)
• Trauma Informed Approach
• Adverse Childhood Experiences (ACE) Awareness Training
• State of Tennessee Child Abuse Reporting Training
• The 2Gen Community Model

Minimum Qualifications
• Master’s Degree in Social Work or Counseling
• Experience: Five to seven years experience in health care, social service or related field.
• Licenses or Certifications Preferred: Licensed in the State of Tennessee as Licensed Clinical Social
Worker or Licensed Professional Counselor with Mental Health Services Provider designation (LPC/
MHSP), or fully licensed in another state with eligibility to become licensed with an aforementioned
designation in Tennessee.
